What is a Geriatric Care Manager?

A professional Geriatric Care Manager (GCM) is certified through the National Association of Geriatric Care Managers.  Many home health agencies utilize geriatric care managers who have not been certified or trained.  GCMs are required to do 2 years under supervision of a licensed professional such as a social worker, physician, nurse, etc.  They must be actively working with older adults and have an advanced degree in nursing, gerontology, physical or occupational therapy or social work.  GCMs do assessments in the home; manage the team that provides services to an older adult; communicates with physicians, dentists and other professionals; maintains status updates with the family, and advocates for the individual at home, in a facility or hospital.  The National Association of Geriatric Care Managers has Standards of Practice which are recommended for all Geriatric Care Managers.  Certification must be updated every 3 years and requires constant CEUs annually to stay abreast of changing practices and information.
